📚 Library Book Borrowing Permission Form

Hello IDCS Families!

My name is Erin Kobyluck and I am the Library Media Specialist at IDCS. We are so lucky to have an incredible selection of books for research and recreational use for students available here at school. However, due to the large amount of library books going missing throughout the school year, we are going to be asking families to sign this waiver giving your child permission to take books out of the Media Center.
